Better control of your diabetes reduces your risk of short- and long-term complications of diabetes. This information includes personal, medical, and some indication of resources available to your to meet your medical/health care needs. You are not required to provide this information. However, if you choose to not provide all the needed information, Your Diabetes Coach reserves the right to deny services due to lack of complete information. Your Diabetes Coach needs access to all of the following information in order to best help you. Your Diabetes Coach, Inc does not sell or share your personal information with any other entity unless required by subpoena or other applicable law other than with two or three marketing partners. We will not share your information with our marketing partners without your prior knowledge and permission, however.
AGREEMENT for SERVICES and TERMS of SERVICE
By completing the Initial Assessment form (and any additional forms that YDC indicates), you are engaging in an agreement to have Your Diabetes Coach assess the completed form(s) and discuss it with you. You acknowledge that this information was given voluntarily and without coercion. You further acknowledge that any time spent on the phone beyond the first interview will be charged at the rates posted on this website at the time of the interaction with Your Diabetes Coach. You further agree to pay for any additional time beyond that included in the original offer for which you have already paid. Your signature on the Privacy Policy/Terms of Service (TOS) Agreement is your affirmation that you have read and understand the terms necessary to utilize the Guarantee (Link on left side of this page).
